# Cohorts

Cohorts are groups of patients targeted by outbound campaigns.

## Creating a Cohort

Go to:

**Outbound Agent → Cohorts**

You can:

* Add individual patients
* Import lists
* Search & filter
* Edit and update cohorts at any time

## Fields Displayed

* Patient name
* MRN / Member ID
* DOB
* Contact number
* Last updated

## Best Practices

* Use specific cohorts for targeted outreach
* Avoid mixing unrelated patient populations
